Anthem/ Blue Shield/Medicare:

We submit claims directly. After we receive payment, you will be billed for any balance due.

Workers' Compensation:

Your employer's insurance carrier will be billed directly. You must provide us with the following information:

• Your employer's name
• Your employer's address
• Your employer's phone number
• The date of injury
• Name and address of your employer's workers' compensation insurance carrier

HMOs:

We submit claims directly. You are required to obtain prior authorization from your primary physician. Co-Pay is due at time of service.

Other Commercial Insurance/PPO:

We submit claims directly. Co-Pay is due at time of service. You will be billed for any remaining balance after your insurance has paid its portion.

Medicaid:

We submit claims directly to New Hampshire Medicaid program. You will be billed for non-covered services.

Liabilities:

Patients covered under liability insurance are responsible for payment at the time of service unless other arrangements have been made.
